Atlantis Urgent Care is offering this new service to allow patients less wait time to see an emergency health professional.

Complete this online registration form to reserve an appointment for the same business day or the following few days.  After pre-registering using the appropriate link below, you must call Atlantis Urgent Care to receive the approximate waiting time if you would like to walk in, or you may reserve an appointment. We begin taking calls ½ hour prior to our opening time.

How does this save time?  By pre-registering, our staff is able to enter your demographic information into our electronic medical record system and verify your insurance information prior to coming in.  Then, when you arrive, we only need a copy of your ID and insurance card(s) and your signature authorizing treatment.

Please remember to bring your drivers license and health insurance cards.
